personal information, you can communicate with the person responsible for the organization to which you address your request. \n<br>\n<br>\nYour request may be addressed verbally to the person responsible, or in writing, but it must be sufficiently precise to enable them to answer you properly. Thus, for example, you can mention the title of the document you are looking for, the name of its author, or the subject treatment. \n<br>\n<br>\nThere is a Directory (R\u00e9pertoire des responsables) that gives the list of the organizations subject to the act, as well as the names of the officials to whom the request should be addressed. This directory is located in the office of Communication Quebec, public libraries, the regional offices of government departments and agencies, municipalities, school boards, health institutions and social \nservices. \n<br>\n<br>\nUpon receipt of this form or for any other written request, the person responsible should send you an acknowledgment of receipt stating the date on which you will receive a written response to your request. The response must be made within 20 calendar days, as per the Act. A possible extension of 10 additional days under certain circumstances. \n<br>\n<br>\nFees for photocopies and transmission of documents could be required.
