This form is put at the disposal of individuals who wish to go to a public body a request for access to an administrative document.
The information that you provide to us in the section will be treated as confidential and will be available only to authorized persons to deal with your request.
If you have identified the document you are looking for, or if you wish to obtain information concerning the law of access to documents held by public bodies and the protection of personal information, you can communicate with the person responsible for the organization to which you address your request.
Your request may be addressed verbally to the person responsible, or in writing, but it must be sufficiently precise to enable them to answer you properly. Thus, for example, you can mention the title of the document you are looking for, the name of its author, or the subject treatment.
There is a Directory (Répertoire des responsables) that gives the list of the organizations subject to the act, as well as the names of the officials to whom the request should be addressed. This directory is located in the office of Communication Quebec, public libraries, the regional offices of government departments and agencies, municipalities, school boards, health institutions and social services.
Upon receipt of this form or for any other written request, the person responsible should send you an acknowledgment of receipt stating the date on which you will receive a written response to your request. The response must be made within 20 calendar days, as per the Act. A possible extension of 10 additional days under certain circumstances.
Fees for photocopies and transmission of documents could be required. Each organization will give you relevant information to this effect.
